Does Your Foot Pronation Really Matter?
Quick answer: Foot pronation is a normal, necessary movement — not automatically a fault. Research on whether “overpronation” causes injury is genuinely mixed. Some studies have explored links between pronation and injury risk, while other research has found no increased injury risk from greater pronation alone. What matters more is how your whole body is loading and adapting, including training load, strength, biomechanics, movement mechanics, footwear, and injury history, rather than simply which way your arch rolls (Nielsen et al., 2014; Agresta et al., 2022).
James had heard it a dozen times before he ever set foot in a clinic. “You're an overpronator,” the assistant at the running shop told him, sliding a pair of stability shoes across the counter. Six months and two pairs of “corrective” trainers later, his shin pain hadn't budged. When he finally came in for an assessment, his first question wasn't about his knee or his training plan — it was, “Is it just my feet? Is that the whole problem?”

James's confusion is common — and understandable. Foot pronation has become one of the most repeated explanations for running injuries, often presented as settled fact. But a 2022 focused review of running injury paradigms found that evidence supporting any single footwear-selection paradigm, including pronation control, is generally limited (Agresta et al., 2022).
So where does that leave you if you've been told your feet are the problem?
What Is Foot Pronation, Really?
Pronation is a natural movement of the foot that occurs as your foot interacts with the ground during walking and running. It helps the foot adapt to the surface and contributes to how forces are absorbed and transferred through the lower limb.
“Overpronation” is generally used to describe a greater degree or duration of pronation than what is considered typical. However, what is “typical” varies considerably between individuals, and pronation by itself does not automatically mean that someone will develop pain or an injury (Nielsen et al., 2014).
The important point is that a running assessment should not treat the amount of pronation as an isolated diagnosis. Foot movement is part of a larger kinetic chain involving the ankle, knee, hip, pelvis, and trunk, as well as the runner's training demands, physical capacity, strength, and running biomechanics.
What the Evidence Actually Says
This is where the topic becomes more complicated than a shoe-shop conversation can capture.
The Case That Pronation Matters
Some research has explored whether greater foot pronation may be associated with running injury risk.
From a biomechanical perspective, it is plausible that differences in foot motion could influence movement and loading elsewhere in the lower limb. However, the relationship is not straightforward.
Pronation occurs alongside other movements throughout the lower limb, and its clinical significance depends on the individual context rather than simply whether the foot appears to pronate more than average (Agresta et al., 2022).
The Case That It's Been Overstated
A widely cited prospective cohort study followed novice runners wearing neutral shoes for one year and found that increased foot pronation was not associated with increased injury risk (Nielsen et al., 2014).
This is important because it challenges the idea that greater pronation can automatically be treated as an injury predictor.
Research on motion-control footwear also adds another layer to the picture. A systematic review found that motion-control footwear can reduce foot pronation and peak vertical impact during landing, but the available evidence did not establish that these changes reliably translate into control of movement in the more proximal segments of the lower limb (Cheung et al., 2011).
More recent reviews of running footwear have also questioned the assumption that one footwear paradigm can reliably prevent injuries. Agresta et al. (2022) concluded that evidence supporting major footwear paradigms, including pronation control, remains limited.
Why the Research Is Mixed
Injury risk isn't decided by one variable.
Training load, recent changes in mileage, pace and hill work, footwear history, strength, running experience, biomechanics, movement mechanics, and previous injury can all contribute to how a runner responds to load.
A foot that pronates more might be perfectly capable of handling a well-managed training plan, while a runner with relatively little visible pronation can still develop pain if training load increases faster than their current capacity allows.

So What Should You Actually Do About It?
Get Assessed, Not Just Measured
A shoe-shop treadmill scan can show you how your foot moves. It doesn't necessarily explain why you're experiencing pain or how your foot movement interacts with your ankle, knee, hip, pelvis, strength, running mechanics, and training load.
A full physiotherapy assessment considers the wider movement picture rather than treating one foot characteristic as the complete explanation.
Prioritise Load Management
If pain has appeared after a jump in mileage, pace, hill work, or training frequency, that change in training load deserves attention.
Rather than assuming that foot shape is the main problem, consider whether your current training demands match your body's ability to tolerate them.
Consider Gait Retraining When Appropriate
A 2024 systematic review and meta-analysis examined gait-retraining approaches specifically targeting foot pronation.
The review found that several gait-retraining strategies showed potential for reducing measures of foot pronation, with changes involving centre of pressure, foot progression angle, and forefoot-strike training showing more favourable results in the included studies (Mousavi et al., 2024).
However, the authors also noted that more research is needed to understand the long-term effects and determine the optimal type and dosage of gait retraining (Mousavi et al., 2024).
So gait retraining can be considered as part of an individual rehabilitation plan — not as a universal instruction for every runner.
Build Strength Where It's Needed
Strength is another important part of the bigger picture.
Calf, foot and ankle, and hip strength can influence how the body manages running loads and movement demands. Strength work can therefore be included in a running rehabilitation plan alongside appropriate training-load management, biomechanics, and movement retraining where relevant.
The goal isn't necessarily to eliminate natural pronation.
The goal is to improve the body's capacity to tolerate and manage the demands being placed on it.
Choose Comfortable, Well-Fitting Shoes
Current evidence does not strongly support one universal footwear paradigm as the answer to injury prevention.
A 2022 review found that evidence supporting pronation-control and other footwear paradigms is generally limited and suggested that clinicians consider factors such as comfort and individual runner needs when selecting footwear (Agresta et al., 2022).
That doesn't mean stability shoes are “bad” or that nobody should use them.
It means footwear decisions should be individualised rather than based solely on the assumption that pronation needs to be corrected.
Recovery Expectations
If pain around the shin, knee, arch, or another area has developed alongside running, recovery time depends on the specific tissue involved, the severity and duration of symptoms, training history, and how well the rehabilitation plan matches the individual's needs.
Rather than assuming that pronation is the sole cause, a proper assessment can help identify the factors that may be contributing to the symptoms and determine an appropriate rehabilitation strategy.
Preventing Pronation-Related Pain in Neath's Runners

Build training load gradually.
Develop the strength and capacity needed for your running demands.
Consider your overall biomechanics and movement mechanics.
Pay attention to changes in mileage, pace, terrain and training frequency.
Don't rely on footwear alone to “fix” your running.
Get recurring pain assessed rather than assuming your feet are simply “wrong.”
When to See a Physiotherapist
Consider an assessment if you notice pain that persists, pain that returns each time you resume running, or symptoms that haven't improved after changing shoes.
A proper movement and gait assessment looks beyond the foot alone. It can consider your running mechanics, strength, training load, biomechanics, movement patterns, and injury history to help build a more complete picture.
FAQs
Is overpronation always a problem?
No. Many people pronate more than average and never experience pain. Pronation becomes more clinically relevant when it is considered alongside symptoms, training load, strength, biomechanics, movement mechanics, and other individual factors rather than being treated as a diagnosis by itself (Nielsen et al., 2014; Agresta et al., 2022).
Do I need orthotics?
Not automatically. Orthotics may be useful for some individuals, but they should not be considered an automatic solution simply because someone has been told they overpronate.
The 2024 systematic review on gait retraining found that gait-retraining approaches can modify measures of foot pronation, although the authors noted that more research is needed regarding long-term effects and optimal protocols (Mousavi et al., 2024).
The right approach depends on the individual's symptoms, assessment findings, goals, and response to treatment.
Should I buy stability or motion-control running shoes?
Current evidence does not strongly support one footwear paradigm as reliably preventing running injuries.
A 2022 focused review found that evidence supporting pronation-control footwear as a universal injury-prevention strategy is limited (Agresta et al., 2022).
A shoe that is comfortable, fits well, and suits your individual needs may therefore be a more sensible starting point than choosing footwear solely because it is marketed as “correcting” pronation.
Can I fix pronation myself?
Some contributing factors — such as strength, training load, and aspects of running mechanics — can be addressed through an appropriate exercise or training plan.
Gait retraining may also influence measures of foot pronation, although the evidence is still developing and the appropriate approach depends on the individual (Mousavi et al., 2024).
